There are several people who have donated via another alumni and hence their information is not archived with us. I urge the connecting alumni to help us in getting the same through this google form. 

It is relevant to mention that we are now an 80G TRUST and hence you can claim tax benefits based on your donation. In order to claim such a benefit, the details requested in this form are unavoidable.
