For taxes, please complete the Form 1040NR, rather than the Form 1040NR-EZ, because you have dependents.  Instructions for downloading Form 1040NR are included in the tax packet.

Dear Erin,
 
This is Xiaodong Wang, a Chinese Fulbright schoolar in Arizona State University now. 
 
As you know, the visa and form 2019 of me and my family members cover period from 09-01-2008 to 06-30-2009.  I plan to leave Arizona at the end of June.  Then I, together with my family, would like to travel to NiagaraFalls, New York and DC for 2 weeks.  Friends suggested that we go across the border to Canada to see the Niagara Falls because the view would be better from their side.  Do you think we may re-enter US with that visa?  Or do we need to extend our visa to July 15, 2009?  What should I do if we need to extend the visa?  If we arrive Niagara Falls before or on June 30, 2009 may I go across the border and be back safely with my visa?  
 
Another question is about my insurance.  I need to extend my insurance to July 15, 2009 whom should I contact with? How much should I pay?
 
The third question concerns the tax Form 1040NR-EZ.  I have independents.  Does that mean I do not meet the first requirement stated under 'Can I Use Form 1040NR-EZ'? In another word, must I use form 1040NR?

Also, I re-sent the copy of my social security card and tax form, sent a letter of explaination. 
 
Would you please tell me when I can get the money? Will the check be sent to my home or ASU or will you send it to the host professor? Anyway, Christmas is the season for shopping.
