Has everyone sent their health forms? What is the status on the TB Screening? I sent them and still have a hold on it. Got a reply saying I need to take them there?

I filled out the forms and got the doc's signature and stuff.
Regarding TB.There is a risk assesment form for TB in there.If your doctor pegs you as a low risk case,you need not take the TB test.But i haven't sent mine in yet,So i am not sure how they would react.

Well my doctor indicated that I am a low risk case. But the admin there said that I am from a high risk country and hence need to take it. But they would not accept reports taken outside of US, I am informed.

yeah...they are not acceptin the TB tests...still on hold 4 me even after indicatin as low risk...
i guess, hav to get it done there....

I sent at 9 PM IST and got it reflected by 10 AM IST the next day.

Have you filled out the health form.What about the medical insurance section?Have you filled it out?What should we write there.Do we have to take out an insurance here?

Nothing if we do not have insurance. We can go and take it there.

Also,what does that "Parent Varicella Infection Confirmation signature"(end of 3rd page) mean?who should sign there?

If you mentioned you had chicked pox in the table at the top of the page, then you parent must confirm this.

I have registration hold in my account as expected.I dint send TB test results and am plaaning to take it once I reach Atlanta.


But what bothers me is the 'x' mark against series 1 of the vaccines Measels/Mumps/Rubella (MMR) and Varicella (VAR).My physician did not fill series 1 since I did not have necessary proof for that.Can this be ignored since I have my series 2 column filled?

In any case recommend having the TB tests and X-ray handy.

Yeah.. tat can b done.. But any idea abt the series 1 of those vaccines? Is it a problem if I dont ve the dates for the tests?

From what I heard they don't ask for some proof for every dose. You can mention the approximate date when you took it.

But my doc is not ready to fill an approximate date in the form :( is there anything tat I can do now?

@janani : Nobody remembers the actual date.My doc filled in approximate date only.Something along the lines of 12/12/1988.It shouldn't be a problem.Why is your doc refusing to fill in an approx date?

I dont know if u ppl got this mail. still, just in case:
It has come to my attention from several of you that the Health Services website says your Health Forms must be FAXED by July 5.  This pertains to undergraduate students.  Graduate Students are not all accepted by that time, and therefore, some cannot get the forms in by that date.  IF YOU CAN get them in by July 5, that is optimal, so do please try to get your health forms (medical history and immunizations) done and faxed as quickly as possible.  HOWEVER, IF YOU ARE LATER THAN JULY 5, FAX THEM ANYWAY -- AND ALSO BRING A COPY WITH YOU WHEN YOU COME TO ATLANTA.  That way you will have instant proof in case your forms did not get processed through Health Services for some reason.  Health Services is using a new process this year to try and speed this along.  Hopefully everything will work out fine. For those of you out of the country -- you must get your TB screening done here after you arrive.  TB screening done outside the US cannot be accepted, so plan to have this done at the Georgia Tech Health Center (or at some other US facility) no later than Wednesday, August 18.  Your records will not be cleared for registration until this is done.

Nope...i don't think so. I didn't fill up the insurance details and there was no issue with the medical entrance form part.

Does anyone know the exact cost of Tb tests at GATECH ? Did they accept the test in India for anyone ?
Just asking to plan my expenses.
