hey guys..am sorry...those guys have not entered dates that i had originally sent them - wonder whats wrong with them! i will send the original dates i sent here...i am sure that these dates are correct as these are verified dates and ramya is right - most vaccines are taken as a kid/baby.
i apologize for my mistake :( but even with later dates u wont have problem except for chickenpox vaccine which was not invented then - so be careful with that date.

@ramya thanks for pointing out the mistake! and yes - dont enter yourself as high risk student for it won't be accepted and u will be made to take the test there in  US.

Reg the TB test. Don't mention yourself as HIGH RISK candidate no matter what - if you are low risk, just fax them the mantoux test report alone - no specific format, just ask the clinic for it. Usually its considered safer to send xray/doctor report as well, but wrt gatech they might assume you are high risk as they have asked for it only if you are high risk. so just send mantoux report alone - if they dont still approve your TB test, then dont mail and ask them anything - just send the xray report, mantoux report and your health forms together this time. Can you also share your Login details with me. I am also confused abt the dates as there are conditions like vaccination shld be 30 days  prior to FASET date etc.
 
Also please tell in detail in detail abt the TB test. just tell in detail what I need to do and send in fax if I am a low risk candidate. If possible please share the xray report format and test report format.

I have sent the screenshot of my IMMUNIZATION LOGIN details to you - only the 2nd series of MMR is not approved. Don't give exact dates..you can find the difference between two dosages and give similar dates. Don't share the screenshots with others - you can share the dates.

I got my TB Screening test approved - go to a clinic, take the mantoux test and get the report for that and also a separate xray report from the doctor saying its normal...fax the two reports along with your health forms and you shd b fine.

heyy arun,
isnt india mentioned as a high risk country??
so wen we say we r from india, that automatically makes us high risk ritee??
so we need to take those tests in the US eh??
and have they removed ur TB hold???

@Kailash yes but i mentioned as not high risk - if not accepted give another shot with xray report. they have removed my tb hold. one thing u must have on mind is u can send the healthforms with diff entries when the hold is not removed ;) they don care. taking any test/vaccinations in US costly..do it as a last resort only.

@arun...oh ok....so wat we do is, put ourselves as low risk people, get both the mantou test and xray done and fax it to them is itt???

@kailash - dont send the xray report now - send mantoux alone now - if they dont approve then send it...its possible that they may assume as a high risk person when u send xray report coz only for high risk persons an xray is asked for..buzz me @ my id if u hav any more questions..
