Hi All,

In the letter from the Graduate Admissions, a link is specified for
purposes of Health Forms that need to be submitted.
http://www.health.gatech.edu/main/10_new_students/. I am unable to
access this link.

Nevertheless I accessed the health form from http://www.health.gatech.edu/forms/GTImmATL10.pdf

Just wanted to confirm if any other important information is specified
in the first link (if any of you able to access it).

Nope,i haven't been able to access it either.I am using the same link
you specified.Think that is the correct one.I contacted one of my
seniors there.They submitted the same form last year.

Hi All,

I have a query regarding the health form that needs to be faxed before
July 5th. I have taken all the vaccines mentioned in it, but do not
remember the exact dates. Neither does my family doctor. Is it fine if
I fill some approximate dates? Pl provide your thoughts.

in this health form, is it ok if all the vaccines are administered in
India or do we need to take the vaccines only there??(cause some TB
vaccine, it is specified u need to take the vaccine up there??)
and could u tell me what all vaccines u hav taken??

Has any1 submitted their health forms? I have and I am still having a hold on my registration. When I mailed them, they replied as below. I took the TB test and Chest X-Ray but they say it should be done there only. Any thoughts?
 
I reviewed your account, the reason you are still on hold is because the TB requirements.
You had the TB done in your country which is high risk so we could not accept that
requirement. You would need to have the TB done in the United States for it to be accepted.

I thought some one in the Gatech Chennai meetup told we can do that from India itself and they will accept.
But yeah, they have clearly stated in the health forms that they shud be done in USA.
Also, I think, ppl were discussing the cost it will take to do them at Atlanta..

Right. Even my friends at GT told me to do them here as it would cost ~$100 there. But looks like they are not accepting them :(

I think it very clearly stated in the orientation information that all
international students would have a registration hold till they
completed the TB screening in the United States. GaTech has arranged
for medical tests to be done during orientation just for this purpose.
So I guess whether we like it or not we have to take the TB screening
test (Manto Test) during orientation.

hey sairam, could u plz tell me how exactly u sent ur immuni forms back? did u print a copy, filled it up and faxed it back? if u can plz mail me a scanned copy of your immuni form..just want to know how to fill it exactly

U need to fax it. Its given in the very first page of instructions.
Even the numbers are given.

U need the signature of the MD who's immunizing you. So u have to
print it, fill it n then fax it.

I have sent the screenshot of my IMMUNIZATION LOGIN details to you - only the 2nd series of MMR is not approved. Don't give exact dates..you can find the difference between two dosages and give similar dates. Don't share the screenshots with others - you can share the dates.

@Sairam, Sangit & Ram,

I got my TB Screening test approved - go to a clinic, take the mantoux test and get the report for that and also a separate xray report from the doctor saying its normal...fax the two reports along with your health forms and you shd b fine.

If any of you have further doubts regarding the health forms like dates/vaccines/tests or want to verify your forms, feel free to buzz me.

I did exactly the same thing as you for the TB test. I sent them the Montoux Test and X-Ray reports. But they are not accepting them. :(

Hi Arun,
Can you also share your Login details with me. I am also confused abt the dates as there are conditions like vaccination shld be 30 days  prior to FASET date etc.
 
Also please tell in detail in detail abt the TB test. just tell in detail what I need to do and send in fax if I am a low risk candidate. If possible please share the xray report format and test report format.

Hi Barani,

I have mailed the my forms screenshots to you.

Reg the TB test. Don't mention yourself as HIGH RISK candidate no matter what - if you are low risk, just fax them the mantoux test report alone - no specific format, just ask the clinic for it. Usually its considered safer to send xray/doctor report as well, but wrt gatech they might assume you are high risk as they have asked for it only if you are high risk. so just send mantoux report alone - if they dont still approve your TB test, then dont mail and ask them anything - just send the xray report, mantoux report and your health forms together this time. arent all the vaccines supposed to b taken by us wen we wer kids? i mean measles,mmr,dt..all those vaccines shud b administerd immediately after birth or within a year after tat!! bt ur form shows everythin given last year..so lil confused..pls explain... btw there is a question in the immuni form asking whether we are in high risk country..wat am i supposed  to tick der? if i say yes, den dey wudnt accept my tb report rite??

i dont think they accept insurance taken in India...
so i guess wel hav to go there and get our insurances..
