Resolve qualified names, stub-declared types, and inherited member types in ToIndexFileConverter

* Resolve a qualified name as a whole, trying each division of its
components into packages and enclosing classes.
* A type that the stub file declares shadows an import and a type on the
classpath, and gets its binary name.
* Resolve a member type inherited from a stub-declared supertype, or an
accessible member type of a class on the classpath.
* A single-type import determines what a name refers to, even if the
imported class is not on the classpath.
* Guess an unresolvable name from the Java naming convention.
