Query SH 7 ROC -Kolkata MOA

We have file Form SH-7 with ROC-Kolkata and following two Queries raised by ROC:

(1) Altered MOA should be duly signed/stamped; 

(2) Scanned images of the documents should be of original signed document - Subscriber's page of MOA/AOA is not the scanned copyof the original, duly filled/signed/stamped.

Whether we have to attach all pages of altered MOA signed and stamp and 

whether we have to original subscriber page hand written at the time of incorporation??
