With just twenty-seven minutes before a bomb explodes in the head of their captured Biotic, Josh, Tiger, and Wolf hold a high-stakes interrogation in Josh's bedroom while Josh's unwitting parents host a holiday dinner party downstairs.

Two adolescents met on a popular dating phone application. One individual had a long-standing history of asthma and peanut allergy. Although they never kissed, the male with peanut allergy received fellatio, while the other male had eaten peanut butter before they met. During fellatio, the peanut allergic male developed respiratory symptoms, used his bronchodilator, and collapsed. He remained unconscious despite aggressive interventions by emergency personnel called to the site. The clinical history and autopsy results suggested anaphylaxis to peanut allergen exposure from the intimate exposure as the cause of death.

We present the first case report of intimacy-related fatal anaphylaxis due to accidental food allergen transfer, after two adolescent males met via a dating application and engaged in oral intercourse. One male ingested peanut butter before the encounter, unbeknownst to the other male, who had an IgE-mediated peanut allergy, allergic rhinitis, and a history of severe asthma. During insertive fellatio, he developed acute wheeze and dyspnea. He did not have urticaria, angioedema, or gastrointestinal symptoms. He used his short-acting bronchodilator and collapsed shortly after developing respiratory symptoms and a personal epinephrine autoinjector was not administered. Forty-five minutes later, emergency medical services personnel arrived and intubated the patient. Palpation and electrocardiogram demonstrated pulseless electrical activity. He received cardiopulmonary resuscitation and multiple doses of intravenous epinephrine. He was then started on infusions of norepinephrine, epinephrine, and salbutamol after return of spontaneous circulation, and was admitted to the local hospital. He received pulse-dosed corticosteroids and bicarbonate. Death was pronounced the next day. The immediate cause of death was pneumonia due to hypoxic ischemic encephalopathy, as a result of cardiorespiratory arrest from anaphylaxis.

The male allergic to peanut was well prior to the encounter and his asthma control had improved on regular medical therapy with clinical control achieved, as per Canadian Thoracic Society criteria, corroborated by improved pulmonary function test parameters at his most recent allergy assessment (Table 1). The patient was followed by Allergy and Respirology. He had avoided peanut and carried an epinephrine autoinjector since he was a toddler for IgE-mediated peanut allergy, initially diagnosed due to immediate urticaria after consumption with an associated positive skin test. Two months before his death, clinical assessment and skin prick testing confirmed persistent peanut allergy with a strongly positive skin test to peanut.

Several other factors support anaphylaxis as the cause of death in this patient, in addition to the elevated tryptase level. The patient had respiratory compromise and cardiovascular collapse after mucosal exposure to a known food allergen, in keeping with a diagnosis of anaphylaxis. A personal epinephrine autoinjector was not administered during the event. Toxicological analysis did not find any evidence of drugs of abuse. Condoms were not used, making other causes of anaphylaxis, such as latex anaphylaxis, unlikely. An additional diagnostic consideration for this case is postcoital asthma, termed sexercise-induced asthma or honeymoon asthma [8]. Parasympathetic and cholinergic over activity in addition to heightened emotions are thought to provoke severe, life-threatening symptoms invoked by sexual arousal, but not by exercise [8]. Symptoms may be pre-coital or occur up to 6 h after coitus and have even required mechanical ventilation [8]. Our patient achieved clinical control of his asthma, as per Canadian Thoracic Society criteria, corroborated by improved pulmonary function test parameters at his most recent assessment. The patient took Salbutamol 1.5 h prior to spirometry which may have improved results; however, pulmonary function parameters were significantly improved compared to prior tests and there was no significant post bronchodilator change. These factors in association with clinical control less than two weeks before his death, make intimacy-induced asthma less likely and anaphylaxis remains the suspected diagnosis.

In 1946, B.J. and Richard Wesley were sent to Castle Wolfenstein within the German Alps in order to retrieve a top secret folder, which contains information on the whereabouts of Deathshead's Compound. Disguised as SS officers, B.J. and Wesley infiltrate and arrive in the castle in a cable car, then continue to the office of Helga von Schabbs. When Wesley finds the safe, but no folder, he tells B.J. to radio Kessler which he reveals that he has new information regarding the folder and that he will meet them at his tavern. After radioing Kessler, Nazi troops interrupt the agents and B.J. engages the troops before Wesley can get a ladder for him to ascend up to the catwalk. As both agents leave the office via a vent, they both are captured by Rudi Jäger, accompanied by his dog Greta and a Super Soldier.

Sent into the holding cells of the castle, Blazkowicz manages to escape with the help of a pipe which he uses as a weapon. He goes through the asylum wing of the castle and evades the jaws of a Panzerhund, as well as the various super soldiers guarding the prison. Blazkowicz manages to secure a boat and rearms himself before leaving for the Docks. Arriving at the docks, B.J. engages various soldiers while opening the keep to the castle. He manages to infiltrate the keep and fight off the various troops guarding the interior of the castle before finding Wesley after being tortured by Rudi in a surgical theater, strapped in an electric chair.

Blazkowicz arrives in Paderborn Village where he meets with Annette Krause and Kessler at the tavern and has to hold off several squads of Nazis before combating Rudi. Upon Rudi's death, Blazkowicz escapes the tavern's destruction as Rudi was armed with dead man's switch (powerful explosives which didn't activate until his death). B.J., Kessler and Annette all leave in a rowboat heading for Wulfburg to meet up with another contact known as Agent Two. Upon their arrival, B.J. headed for Agent Two's location at a baker shop and finds that it's Pippa Sheperd who is his contact. She tells him Helga is at the Wulfburg Tavern where he heads to and poses as a waiter in order to get close to Helga and leave with the folder.

While trying to leave with the folder under his tray, B.J. is stopped by Helga and ordered by her to taste the wine. After she tastes it, Helga is outraged by the fact that the wine wasn't to her satisfaction and orders Blazkowicz to drink it. B.J.'s cover is blown when Helga takes her kitchen knife and stabs him in the hand and orders Emmerich Schreiner to keep him covered. Blazkowicz manages to pull the knife out and stabs Schreiner with it and obtains his Handgun before he is hit by debris from the ceiling caused by an earthquake. B.J. gets up and heads back to Pippa, but encounters several Shamblers along the way.

Upon his arrival, Blazkowicz witnesses the death of Pippa at the hands of a shambler and kills it before obtaining his weapons. At the same time, he also saw Pippa come back as a shambler and killed her before leaving. Engaging more undead along the way, B.J. finds himself utilizing a Laderoboter to get him passed various obstacles in the way before arriving to the dig site. B.J. arrives at the dig site and continues into the caves where he finds Von Schabbs and Schreiner in the chamber housing The Monstrosity. B.J. combats the monster and kills it, then heads to see a wounded Helga and obtains the folder. Before leaving, he is buried underneath several rocks, but is rescued by Fergus Reid and other allied troops before heading to R.A.F Kinloss where they board an aircraft and head of Deathshead's compound.

In 1946. B.J. and the Allied soldiers launch an assault against Deathshead. B.J. is knocked unconscious countless times during the fight but is revived by Fergus and keeps going for the visions he sees of a safer, happier life. After gunning down multiple Nazi aircraft, one of them crash lands into the turret area destroying it. Fergus is radioed by Condor 9 to investigate issues with their fuel tank, with the fuselage hit and leaking. Condor 9 thanks fergus before a piece of shrapnel kills the pilot. A soldier on the troop transport, Private Wyatt, is instructed by Fergus on how to stop the transport from nosediving into the water. The situation is handled, with Fergus boarding the plane, abandoning his own. B.J. just barely makes it before the plane crashes and cuts into the transport. Fergus takes control of the transport, and B.J. is able to calm Wyatt's nerves. This doesn't last long though, as Fergus makes a crash landing on the beach. B.J. lands in the water but is flung out by a Panzerhund, luckily into a crashed plane giving him cover from the beast. Contacting Fergus via the phone inside, Wyatt flashes his light to give B.J. their location which attracts a Panzerhund to them. In order to save them, B.J. is instructed to push to a nearby turret. B.J. narrowly avoids a Panzerhund as he swims through the water, eventually destroying the Panzerhunds pestering his squad. However, they can't make progress with the Automated Machine Guns keeping watch over their position, and B.J. is sent straight down the middle into the enemy trenches in order to eliminate the pill boxes lining the perimeter and his team reunite and eventually climb to the castle.
