Missing reports

Dear All,

What could be the reason for a missing hostname report? (unreported website)

Is it mostly due to CT inability to cover the whole internet, defunct websites, or maybe no encryption from the domain's side (unsafe site)? 

Moreover, in case the data exists, does it necessarily cover the domain's history since initiation?
