SMART on FHIR - PowerChart vs Custom URL

Hi all, 

We are working with a health system to enable a SMART on FHIR launch from Cerner, with the intent to launch our application in a separate browser.

We'd like to have the system pass a claim like the user's Cerner User ID when launching the SMART on FHIR link to initiate our application. 

Our health system partner has shared URLs that look like this: 

"https://smart.cernerpowerchart.com/smart/apps"

We don't understand how they've gotten a link like that, as our SMART on FHIR link is: 

"https://web.currenthealth.com/auth/smart" 

Questions: 

1. Why does our link look different than what our health system is expecting? 
2. How do we advise the health system to configure the tokens/claims passed alongside the SMART on FHIR launch from Cerner to include things like the Cerner User ID? 
3. Is there a list of tokens that Cerner supports as part of the SMART on FHIR launch?
