R4 Removal of Bundle.total from search transaction responses

In order to increase the consistency of various APIs exposed through Cerner’s FHIR server, we are planning to remove Bundle.total from a few resources. At this time the planned change will affect:

• AllergyIntolerance
• Condition
• Procedure
