Hello,

I am evaluation Cerner Millennium capabilities to integrate progress notes from our voice based app. Basically I want to understand through what mechanisms is impossible to push in a progress note for a given patient's appointment/encounter into Cerner Millennium?
We want the integration to populate the appropriate sections of the progress notes (History, HPI, Allergies, Medication, Physicial Exam, ROS, Assessment, Plan etc.) rather than as a separate document  or attachment to an encounter - i.e. the note should look like it was directly created in the EMR.

It doesn't seem like FHIR is an option here. What about HL7 or non-FHIR APIs? Is there a different forum to discuss those mechanisms?

Can someone guide me through this? Thanks a bunch in advance

Hello Anish,

On my app, we enable the end user to create a Progress Note by voice command using the FHIR API.  Like you, I initially designed the capability to document the various parts of the Progress Note (Subjective, Physical Exam etc.. )  the issue is that the APIs that are with FHIR aren't designed to read or call a part of the note, the API calls and reads the entire note, and therefore we allow the end user to dictate the whole note with heading and so on.  I would recommend you doing the same, most Providers would feel comfortable doing that.

I am not sure that Cerner has specific APIs that do this, and i know Epic and Allscripts dont.  I can say that when you view the dictated note in the system, it looks like a completed document.

Thanks Carl ..this is super helpful! just out of curiosity what FHIR endpoint are you using to push in the note in entirety? That's actually what I am looking for as well ...some EMRs have this concept where things go in seperate sections but for Cerner based on your comment it sounds like it's possible to push in the entire note as such.
You seem very experienced with this ..if you don't mind i'm going to connect with you on LinkedIn.

Thanks,
Anish

Anish,

In FHIR, this is done via DocumentReference create. You can see the documentation and an example here: https://fhir.cerner.com/millennium/dstu2/infrastructure/document-reference/#create

Jenni
