Is there already an API endpoint for retrieving an entire encounter in CDA format - such as a CCDA ?

Peter, we don't have that available currently, but it is on our plan for the year to deliver as we work towards meeting Meaningful Use, Stage 3 requirements.

Will it be as a FHIR document or in "normal" CDA format?

Shamil,

Meaningful use calls for a normal CDA document to be returned - likely via the Binary FHIR resource.

Yes, exactly.  I wasn't expecting it to be part of the FHIR resource list when I posed the question, I was just hoping to find it somewhere among the published APIs.   After all, isn't it a requirement for MU2 compliance?

Under FHIR, implementing the heterogeneous Bundle resource could, in a small way emulate the CCD.  Even better than the CCD would be something modeled after the QUICK specification.

MU2 required that the CEHRT be able to produced a CCDA, not make available via an API. So systems must prove they can produce and transmit C-CDA, but those transmissions are done via direct or XDS.
