Through what mechanism or interaction are Practitioner resources added as participant members to a CareTeam resource?

If a patient has had a past encounter with a Practitioner, will that Practitioner then be added to the CareTeam resource associated with that Patient?

Are there any other ways to determine if a Patient has had a past encounter (or appointment) with a Practitioner? I think it is possible to historically search past Appointments with status=fulfilled but thinking this may be excess load on a system?
