DocumentReference does not return Total

Hi 

The document reference documentation (https://fhir.cerner.com/millennium/dstu2/infrastructure/document-reference/states) includes the “total” field in response. However, the response when calling the API (https://fhir-open.cerner.com/dstu2/ec2458f2-1e24-41c8-b71b-0e701af7583d/DocumentReference?patient=12724066&created=ge2020-01-01&created=le2020-12-31) does not have a “total” field in response. 

Does DocumentReference response JSON return 'total' or not?

Hello,

While this is a known issue, 'total' is returned when the response contains a single page. If you get paged results, 'total' won't be available.

The long term strategy is to remove the total field.
