We have a web-based app (not SMART) that uses Patient Demographics to query a FHIR server and return FHIR Resource Bundles. We currently do this with EPIC, etc.

Can we access Cerner Millennium without a SMART app?

Yes, you can just leverage the FHIR APIs without SMART.
