Immunization entered-in-error

Hi,

  we are seeing entered-in-error Immunization without a date nor a vaccine code:

(this is from  Jackson Memorial Hospital). Not having the vaccine code nor the date we cannot find out if this is a vaccination record we got previously - and so that should be removed as in error, or if it is something new.
