Does creation of this resource result in an email being sent to the mailbox associated with the Practitioner being referenced? Or is it a notification?

Does the document or notification display in the chat of the Patient being referenced?

New messages via the Communication resource will be sent to the Practitioner's or pool's inbox in Message Center. The category [1] will determine the type indication on the message.

Is there a way to 'Addend' to an existing message vs creating a new message?
