CareTeam resource relation

Hi Team, 

We understand that CareTeam is a list of members involved in the CarePlan realization. The CareTeam is a CarePlan specific. In FHIR DSTU 2 the participants (who are basically, the care team) are incorporated into the CarePlan resource. 

Meanwhile, in Cerner FHIR DSTU 2 the CareTeam is separated from CarePlan resource. And we don't see the relation between those 2 resources. Could you please help us to understand how to identify the appropriate CareTeam for the CarePlan given?

Hi Anna -  Our current implementation doesn't support the ability to identify a care  team for a given careplan.  The care team is specific to an encounter or across all the patient's encounters (longitudinal).
