ADT Information with FHIR

Hi there - 

Primarily we were looking to see if there is any availability of ADT (Admit Discharge Transfer) events from within FHIR - via a hook or event notification. We see a potential way via implementing a patient-view CDS Hook and then querying the relevant recent Encounter(s). Is this the current recommended approach?

Hi Kunjan.

This approach may work; though, it would be better to have a hook specific to a discharge event. However, each EHR likely has varying capabilities for this particular workflow.

Without knowing the specifics of your intended functionality, it is difficult for me to give a good recommendation for a path to pursue. What specifically do you want to do when you are informed that a patient is going through the discharge process?

Thanks. We need to look at Admits. One particular use case is to look at Medications currently active on admission.

Hi Kevin,

What we are trying to do is look at medication-prescribe CDS hook. We want our application to be notified of medication changes at point of prescribing. Or if that's not possible soon after. Looking at one previous discussion in this mailing list on that topic looks like Cerner is waiting on the spec to reach 1.0. Is that still the case? Can you shed some light on that?

Correct, we will release our CDS Hooks support after the release of the 1.0 specification. However, we can't share any specifics beyond that at this point.

I'm looking for more information on the availability of CDS Hooks as well.  Particularly, a hook that would know when a certain test is ordered.  Are we still waiting for the release of the 1.0 specification?  If so, is there an ETA for the 1.0 specification release?

HL7 controls the timeline of the release of the specification. We cannot comment on when HL7 plans on releasing the specification. You are certainly invited to participate in HL7 events to help further the specification, and contribute to its development! Check out cds-hooks.org to learn more. 

When we are closer to releasing CDS Hooks to a Cerner environment we will notify the group of its availability.

HL7 has released a 1.0 spec for CDS Hooks: https://cds-hooks.org/specification/1.0/

I don't have any timeline information to share with you on when Cerner's implementation will be available per group guidelines.
