Populating and keeping patient dashboard in sync

Hello!


We are currently leveraging our system to integrate with PowerChart. Primary, we are integrating our medic dashboard as a provider application. Briefly, this dashboard provides the caregiver with a list of its patients sharing the same clinic and condition. The patient list is ordered based on the current health status.


Each patient will also be assigned access to our patient application, which of course will not be integrated with power chart.


To cover this use case, we need to be able to keep the cohort of patient that a given caregiver has permission to view, in sync with PowerChart. This most be a use case that you have seen before? PowerChart should be the single source of truth and all other depending system should reflect that state. Otherwise, managing patients and permission will be a hassle for the end user.


I have been scanning the forum and the result seems that it is not possible to do this.

What would, today, be the best practice to solve this problem? Since there are not way to fetch all patients, will the registration step be manual for now? I.e. manually register the PowerChart patient id with a user id in our system.


Also, how do you suggest that we solve the initial registration phase for a caregiver? The first time they launch our system we can ask PowerChart for the profile. Is there any information in the provider meta data that indicates what customer it belongs to? Or will this also be manual?

Hi,

For the patient identity side, you can have them sign in with their existing credentials/portal account which can then be used to access the patient data via FHIR. How this is tied to an identity in your application may vary based on your architecture/design.

As far as getting the list of patients: you are correct that we don't currently provide access to a list of patients that a provider has maintained. We DO enforce that the provider cannot access a patient they don't have access to when signed in and using tokens on behalf of a provider. Most often, these lists (when we do expose them) are based on recent visits rather than the entire history of the clinic - so it depends on your use case. 

As of now, our APIs are built more for day to day workflows of a provider accessing the patient chart in realtime (or a patient getting access to their data in realtime). An example of how to see a view of patients that the provider would be seeing would be to search appointments that the practitioner has that day (practitioner id would be returned as part of the SMART context): https://fhir.cerner.com/millennium/dstu2/scheduling/appointment/
