Hi, 

Can we get some clarity on what fields/set of fields in the R4 response distinguishes a historical medication vs. a discharge medication vs. an administered medication.  

According to Cerner's reference page when both are present (Reference pages.PNG attached below) we can assume a medication is historical or discharge. What about administered? 

Also, can we assume that both sets ("discharge & community" and "patientspecified & community") will always be in the R4 response? 

In some cases the system is different too (R4 response.PNG attached below)? Is there any logic behind this?

Hi Victor,

The followings are the answers to your questions.

Administered medications are NOT returned in MedicationRequest. Those are returned in MedicationAdministration which is not yet available in R4.

Yes, for Prescription/Discharge order, the "discharge & community" will always be in the R4 response. For Documented/historical med, the "patientspecified & community" will always be in the R4 response.

This is because discharge is only in the value set medicationrequest-category https://hl7.org/fhir/r4/valueset-medicationrequest-category.html while patientspecified is only in the value set medication-statement-category https://hl7.org/fhir/r4/valueset-medication-statement-category.html
