I created a form for the Music Dept., in Google Docs, to enable high
school students to request audition material and an audition date.

That's a great idea, Craig. Where does the form live? on a web page?
or do you mail a link to prospective students?
What system does it replace?
Do you have it set up so that you are notified by email when anyone
submits anything? (Share/set notification rules)

Before the form was in place, prospective students inquired by phone
or email, but we found that they frequently didn't provide all the
information we needed. The form is accessed via the auditions page on
the music dept. web site. Our admin assistant is notified by email
when somebody submits a form.
