Primary culture of mouse skeletal muscle cells

I had been trying to initiate primary cultures from mouse skeletal muscle cells. But not happening properly. Facing the following problems:

1. Skeletal muscle is not completely disaggregating into individual cells. I can see lot of undissociated mass and fibres. May be enzymatic digestion is improper. Could you please suggest a good enzyme recipe and, digestion time?

2. Cells and even the explants are not at all adhering to the flask (using Nunc treated flasks). Do i need to use collagen-coated surface compulsorily? 

And also if you throw some light on the growth medium to be used for the purpose would be really kind.
