Just to follow up, after you get the CPSO#, you get your license in the mail within 4-7 days depending on where you live (I live 40 minutes away from the CPSO building and I just received mine today even though I got my license number on Monday!!). In the package is a form for change of address which I suspect most of us would need as we move closer to our universities. Also attached is the Declaration form for the professional liability coverage and both of these need to be mailed/faxed/ scanned and emailed (
membe...@cpso.on.ca) as soon as possible. I called up CPSO to ask them if I still need to send the Declaration since I already sent them the email with the updated CMPA coverage. They said they still need to have the Declaration form filled out for their records which is not short of what I expected since we all know how much they love their paperwork. For the mailing address change form, they said I can just send an email to them (
membe...@cpso.on.ca) specifying the address change and the effective date for the same but I am sending the form just to be on the safer side.
