Reminder about submitting clinic documentation to School of Educ in Taskstream

If you were enrolled in ANY clinical practicum (in-house or externship) for fall 2011, spring 2012 or summer 2012, you MUST submit your clinical documentation into the SoHaNS DRF in Taskstream according to each tab/category. What is required—electronically--is
your clinical skills eval form; a sample of a semester summary (in-house) or progress report (externship); and a sample SOAP or other therapy log.
PLEASE do this asap if you have not already done so. Our program is part of the School of Education’s accreditation application, and the entire assessment standard of the application is to be reviewed by the accreditors through Taskstream.
In case you have not enrolled, the instructions are attached.
