Revised questionnaire

The order of questions haven't been checked.

The questionnaire is pretty good. But i have a few points to note:

The second question should be 'If no, please proceed' 

 I am not sure whether 'please hand the questionnaire back to interviewer' should be stated as from my part it is given that interviewers will speak out the questions to interviewees
