I went to a meeting where i learned that it is estimated that rebates help insurance companies keep premiums low for their clients, about 1%, which is huge considering the size of that industry.

 Even if competitive open or generic insulin reaches the market (wallmart's insulin does not seem to be as godd, isn'tit?),  it will not be favored by pharmacy's benefits managers.

Pharmaceutical companies seem to love this reform, probably because it doesn't affect their profit margin which i believe is extremely high already, but i couldn't get a number on this.

There are certain older insulins (NPH from 1950, Regular from 1982, and 70/30 from 1989) which Wal-Mart sells OTC for $25.  Regular Insulin is slow acting which can work OK for very low-carb high-protein meals, but NPH is simply the old-school crappy version of glargine that would probably be off the market in an ideal world.  70/30 is an insulin which mixes the two. 

They’re not great options, but I suppose it’s better than NOT having any OTC options for a life-saving drug.
