CCDA templates were built in "layers". In certain places, a less restrictive section template was created that did not require the information in the section to also be represented as machine processable entries. Those types of section templates often have the term "Optional Entries" in the name. For implementers who support including the machine processable entries, they can assert conformance to the "tighter" version of that template. The tighter version of the template goes by the same name but includes "Entries Required" in the name. The tighter template has all the same constraints as the looser one in terms of the requirements on the section.code element and the section.title and the section.text. The template purpose is exactly the same and It uses the same types of entry templates for representing the information. The only difference is that the requirement for including the corresponding machine processable information is a SHALL rather than a SHOULD or MAY.
When templates are designed in layers like this, the tighter template still conforms to the rule established by the looser template. In essence you can always substitute the more tightly constrained template for the looser one. You don't need to include them both. Also note, you can not substitute the looser one when the tighter one is required with a SHALL.
