Seeking OER Needs Assessment/Gap Analysis Planning Documents

Hello, 

We are gathering a group of instructors across a few departments to examine their courses, which are currently using traditional publisher textbooks, to support them in converting to OER (and/or ZTC). 

I'm wondering if there are Needs Assessment and/or gap analysis planning documents that others have created and would be willing to share. I'm thinking of a worksheet-type document where faculty would be able to document what they are using, what they've found for OER and what they would ideally need or want to make their class OER.
