关于办理护照及签证的回复

华协会员，

前一段时间华协有意组织会员集体办理签证及护照，以便于邀请芝加哥领事到本地现场办公。非常不幸由于人数及长周末的原因，领事馆不能在July 4th 期间到GR现场办公。不过，如果不是急件的话，川菜坊的俞圣怡（CHRISTINE）女士有意为大家代办($20代办费)签证以免去大家的芝加哥之旅及回邮费用。护照，认证（Authentication）,及健在证明需要本人亲自办理。

另： 本周日（Jun 29）领事馆在St. Joseph, MI 现场办公，认证和健在证明可当场办理及取件。签证及护照可提交申请及办理回邮。

注： 领事馆规定，申请表必须是电脑打印，不接受手工填写表格。

签证VISA Application Process (for Mainland China):
Accepted forms of application
不接受邮寄申请。一般情况下，申请人可亲自递交材料，或委托他人递交材料（无需出示委托书，一人可为多人递交材料），无需预约。
Visa applications should be submitted by the applicant or someone else
entrusted in person (French and Turkish citizen need to come in
person). It cannot be done through mail. No appointment is required.

Process fee
The process fee for American citizen is $140 per person. If you are
not applying for the Chinese visa in the native country of your
citizenship, the process fee depends on the number of entry (Single
entry: $30. Double entries: $45. Six months multiple: $60. One year
multiple: $90). The consular officer will decide on whether or not to
issue the visa and on its validity, duration of stay and number of
entries according to the specific conditions of the applicant.

Accepted forms of payment and process time
You can use credit card (MasterCard, Visa), money order and cashier's
check for pick-up. Please make the money order or cashier's check
payable to Chinese Consulate in Chicago. Applicant will be given a
pickup form when submitting the application. Applicant or someone else
can pick up the visa by submitting the pickup form. No documents can
be released without this form.
1) Regular Service: 4-5 working days from the date of submitting the
application.
2) Expedited Service: 2-3 working days from the date of submitting the
application and the extra $20 will be charged for payment.

The same day service is not available.

Applicants who cannot come back to pick up could request mail back
service. No Expedited Service is available. The package will be mailed
out in 7-10 working days from the date of application. Please submit a
money order/cashier's check (add handling fee of $5) and a
self-addressed pre-paid return envelope with sufficient postage when
you apply. Only the following return envelopes with tracking
number/bar code are accepted: UPS (air only), FedEx (air only), and
USPS Express Mail. Certified or registered return envelopes are not
accepted. Please make the money order or cashier's check payable to
Chinese Consulate in Chicago. Applicants are solely responsible for
the correct address, postage provided on their return envelopes, any
loss or damage to your passport and documents due to any mishandling
by the post office or other delivery services.

Effective January 1, 2014, the Visa Office will no-longer accept

申请房屋委托书公证材料:
委托出售、转让房产：填妥的《公证、认证申请表》、有效的中国护照原件及带照片资料页复印件、有效的在美合法居留证件原件和复印件(如绿卡、工卡、I-20、H1B、L1、J1等，B1/B2不办理此业务)、委托书文本、房产证复印件或购房合同复印件，若是婚后购买的房产应提供结婚证复印件并由夫妻双方共同申请办理。如委托书上注明身份证号，必须提供申请人身份证原件。

申请房屋委托书认证材料:
委托出售、转让房产：经州务卿认证过的委托书原件和复印件、填妥的《公证、认证申请表》、有效的中国/美国护照带照片资料页复印件、有效的在美合法居留证件复印件(如绿卡、工卡、I-20、H1B、L1、J1等，B1/B2不办理此业务)、委托书文本、房产证复印件或购房合同复印件。

申请结婚证认证材料:
经州务卿认证过的结婚证原件及复印件、填妥的《公证、认证申请表》、有效的中国护照带照片资料页复印件、有效的在美合法居留证件复印件(如绿卡、工卡、I-20、H1B、L1、J1等)。

申请小孩出生证认证材料:
经州务卿认证过的出生证原件及复印件、填妥的《公证、认证申请表》、父母一方有效的中国护照带照片资料页复印件、父母一方有效的在美合法居留证件复印件(如绿卡、工卡、I-20、H1B、L1、J1、B1/B2等)。

申请无配偶声明书公证材料:
填妥的《公证、认证申请表》、有效的中国护照原件及带照片资料页复印件、有效的在美合法居留证件原件和复印件(如绿卡、工卡、I-20、H1B、L1、J1等，B1/B2不办理此业务)、声明书文本。如声明书上注明身份证号，必须提供申请人身份证原件。

申请寄养委托书公证材料:
父母双方须到场，请携带填妥的《公证、认证申请表》、有效的中国护照原件及带照片资料页复印件、有效的在美合法居留证件原件和复印件(如绿卡、工卡、I-20、H1B、L1、J1等，B1/B2不办理此业务)、小孩出生证原件及复印件、委托书文本。如委托书上注明身份证号，必须提供申请人身份证原件。

申请放弃继承权声明书公证材料:

为中国公民在美国所生儿童办理旅行证的常遇问题解答：
1小孩在美出生时，父母一方或双方已有绿卡，或一方为外国国籍，请为小孩申请签证。

注意：为小孩第一次申请签证，还须提供小孩出生证复印件、父母一方的的中国护照复印件和美国绿卡复印件，若父母一方为外国国籍须提供外国护照复印件。
2小孩在美出生时，父母双方均无绿卡，请为小孩申请旅行证，但如果曾为小孩办理过签证，则继续申请签证；父母已经给小孩办理过旅行证，之后获得绿卡但期间从未给小孩办理过签证，则继续申请旅行证。

注意：1）儿童父母双方或一方须携儿童，根据在美居住地，按中国驻美使领馆领区划分，亲自到使领馆办理。如父母一方不能前来，还须提供由其签署并经当地公证机构（如在中国境内，须经中国公证机构）公证的同意由配偶为该儿童办理旅行证赴华的书面声明原件。若父母一方不在美国境内且不能提供护照原件，还须提供护照公证
（须为每一页公证）。
2）若父母离异，小孩监护人还须提供离婚证和法院判决书复印件，亲自带小孩前来申请旅行证。
3 若父母一方为中国国籍，另一方为外国国籍，小孩在中国出生后持出入境通行证来美，在美期间从未办理过签证，请为小孩申请旅行证回国。
