Dear Sir/Madam,

Pls see attached forms required for activation of Pipped Gas. The
attachments are.
1. Aditya Gas Systems Activation Form. - 01 Page.
2. HPCL KYC Form - 02 Pages.
3. Format III - 02 Pages.
4. Annexure  1 - 02 Pages.

Along with duly filled hard copies of these forms, other attached required
are.
1. Photograph - ID size of applicant.
2. Identity Proof - Pan Card or Drv Lis or Voter Card or Passport.
3. Resident Proof - Index II or light bill.
4. Cheque in favor of "Aditya Gas Systems" of Rs 3000. which includes Rs
2000 refundable deposit and Rs 1000 activation charges. Suraksha pipe (1.5
mtr) shall be installed by us.

Flat Holders need to submit the hard copy of this whole set, duly stapled
in Sales office of Casa Imperia, from where we shall collect them and give
connection. We shall require atleast 60% of forms ie 85 forms for (Soc B)
LPG approval & activation.
