model not found in clean

Has anyone seen this before?
Sometimes when I try to image (for this example imagename="test")
clean errors WARN with "test.model not found" and then throws an
"Image is detached". It doesn't always happen!
When it does it happens almost right away, usually just after "Created
and initialized mask mask0."
I suspect this might be a filesystem error, since it doesn't happen
all the time. So I thought I'd cast a wider net before I file a
ticket. Ring any bells?

Don't know...but this sounds like the ia tool complaining ...and ia is
used in the task to regrid a model on disk to the image size requested
in clean.

Right except in this case its regridding a blank model image that
clean just created.

Dear Sir,

       I'm K.Hariharan, graduate student at the Indian Institute of Astrophysics, Bangalore. I'm trying to use our Heliograph (GRAPH ) ,a T- shaped interferometric array, data for imaging in CASA. 

          Using the task importuvfits I'm able to generate a measurement set (.MS) file from an AIPS compatible fitsfile, with a few warnings.

But when I use the subsequent task like listobs I get following errors

2013-09-21 14:07:34    SEVERE    listobs::ms::summary+    Exception Reported: MSMetaData::_getObservatoryPositions: The name of the telescope is not stored in the measurement set.
*** Error *** MSMetaData::_getObservatoryPositions: The name of the telescope is not stored in the measurement set.

   And with the task setjy I get the following messages

        And I'm unable to go further with bandpass/gaincal tasks. I've attached, herewith,  the log file for your reference. I kindly, request you to have a look at it and help me figure out what the issue could be.  Thank You.
