This is a reminder that the HMIS team will be separating Prevention and Rapid Re-Housing into their own programs in the next couple of weeks.  If you and your staff have not yet reviewed the Housing Status at Intake for all consumers, please do so immediately.  Please remember that Literally Homeless will be Rapid Re-housing ... and that status applies to all consumers who were within 14 days (documented) of losing housing

 

Unstably Housed/At Risk of Homelessness will be Prevention

 

Stably Housed/Don't know/Refused will be put into a separate holding program because they are not eligible for ESG (including STEHP) funding.

 

Sue or I will be contacting each program with the lists of consumers for verification and to let you know exactly when your programs will be split.  Once the split occurs, users will have to make sure that they put consumers in the proper programs moving forward.
