If you fly either the club gliders and/or tow plane or plan to start flying please let me know ASAP the following information. Please respond directly to me and do not reply to all as this is a large group of people that I doubt you want all the information going out to.

2. Age

3. Ratings

4. Date of Last Medical and Class or BasicMed (if glider only no medical is required)

5. Any loss that you have had in the last 5 years.
