I am sharing translated versions of the Client Privacy Agreement (Spanish, Arabic, Pashto, Chinese). I'm also re-sending the English, so you can see the "Agency Name" sections highlighted and add in your agency name in the correct spot in the translated versions.

Dear Food Security Network Partners,

As you have seen, we are rolling out a new Agency Agreement with all of you. Part of this includes a requirement from Feeding America to have more formal procedure and agreement in place regarding data collection and sharing, and user confidentiality, and recording client consent.

Hope Clinic and Manchester Community Resource Center graciously agreed to go first rolling out the new data sharing agreement, user confidentiality form, and logging confirmation of client consent in Link2Feed. Thank you!! The feedback from Hope Clinic and Manchester Community Resource Center was that it went very smoothly!

We are leaning on the capacity and expertise of our Food Security Network to go next in rolling this out, before we expand it to include the full network of partners using Link2Feed.

What's next? Please complete the following highlighted steps. 

1) Data Sharing Agreement - please sign and return pages 1 and 2. (Page 3 is an example of the User Confidentiality Agreement that your organization will hold onto, and Page 4 is the annual certification you will send us in May. We'll remind you about that later.)

We will sign it after you return it to us, and then send back the completed agreement for your records. 

2) User Confidentiality Agreement - we are giving you a template to use. Please have all staff who will access (enter, see, or use) client data in LInk2Feed complete the form for your records. Your agency will store these agreements. You don't return the user confidentiality agreement it to Food Gatherers.

3) Client Privacy Agreement - please print and have visible for clients (this is what they are agreeing to when they check the consent box on the Link2Feed form Larry shared).

4) The Implementation Checklist helps you keep track of these steps! 

Please review the Implementation Checklist, which has a screenshot of the new field in Link2Feed so you're team is familiar with the change. Recording client consent is done once, and does not need to be asked every time.

5) The Script and FAQ are available if you find them helpful in answering questions from clients.

6) Updated Link2Feed intake forms w/ the checkbox in English, Spanish, Arabic and Mandarin.

7) Updated Link2Feed user guide

We are planning on turning on the "client consent" checkbox on Link2Feed on Friday March 13 - Monday March 16.
