Try sliding the buttons sideways in portrait mode. Maybe the buttons are hidden. There are three sets of buttons available to provide lots of keys for complicated games.
Unfortunately, there is no provision to remap the Wii controller to keystrokes. It has a fixed function as a joystick and the buttons are mapped accordingly. I'll mark down your suggestion for remapping of the Bluetooth controller to keystrokes.
I use candy apple on my nexus 7 and it works OK so we should be able to get some buttons going for you in portrait mode.

Are you able to map some buttons in the portrait mode? Could it be that there are just no buttons mapped there right now? If you can map some, do they still show up when candy apple is restarted? The portrait and landscape buttons are stored per diskette in the same sqlite database. Additionally, the most recent button mappings are stored in android preferences. BTW, the landscape and portrait buttons are not linked in any way so it is possible to have some buttons mapped in landscape and not in portrait.
Please let me know if you are unable to map any buttons in portrait mode.

The Android OS decides what file the prefs are stored in so I can't guess which one is active for you right now. It has been a while since I coded that but I believe that what is stored is the character as well as the keystroke value. I could take a look later if we can't get this resolved.
This really sucks that you are having so many problems with these buttons. I've never seen this problem before. I'll have to think about what could have gotten us to his state. If I had this problem I would uninstall and delete the stored SQL database etc. But you've already tried that. Hmmmm.
Let me think about it some more.

OK, I went back to my Nexus 7 and it has the same problem! I have to go to work :-( but I suspect that it just can't display the buttons for some reason. This all used to work on the n7 but some upgrade from 4.1 to 4.2 jelly bean must have triggered this issue. I'll try to fit in some work on this over the weekend.
Sorry you had to find this the hard way.
