Cancer Report crashes the validator

I read the other thread(s) that discussed this; but even after going through schema validation AND no-test-case validation, the tool crashes when I select the first test case.

I'll gladly send the file; had hoped I could attach it here  (maybe after I post it?)

The Validator does report one error that I can't fix; it asks for a code from the Healthcare Service Delivery Location value set that no longer exists in that value set.
