Upgraded to Rails 3.2.8

Calagator is now running on the current release of Rails:

* Upgraded Calagator from Rails 3.0.17 to 3.1.8 to 3.2.8.
* Upgraded things broken by API changes.
* Upgraded plugins, locations and loading to new conventions.
* Worked around incompatible "loofah-activerecord" gem by making the changes myself and submitting them to the author.
* Worked around incompatible "ri_cal" with monkeypatches, since it has joined the proud ranks of abandoned Ruby calendaring libraries.
* Worked around incompatible "validation_reflection" gem by finding a fork.

Sad to hear that the state of Ruby calendaring libraries continues to decline :/
