There is a side quest, though. That is probably the cultist you are refering to. At one point a cultist really wants to try some poop, and when you feed it to him, it increases his loyalty towards you. In this single case, I would recommend feeding your cultist poop.

Cult of the Lamb: Cultist Edition bundles the base game with the Cultist Pack which includes a seven-piece decoration pack for your charming commune, as well as five exclusive Follower forms. You'll be the envy of every cult.

Cult of the Lamb casts players in the role of a possessed lamb saved from annihilation by an ominous stranger and must repay their debt by building a loyal following in his name. Start your own cult in a land of false prophets, venturing out into diverse and mysterious regions to build a loyal community of woodland worshippers and spread your Word to become the one true cult.

SPREAD YOUR WORD
Train your flock and embark on a quest to explore and discover the secrets of four mysterious regions. Cleanse the non-believers, spread enlightenment, and perform mystical rituals on the journey to become the mighty lamb god.

As you begin to build up your cult, you'll be able to indoctrinate a variety of followers, but you'll have a bit of customization to choose from when selecting your next follower. While potential cultists all come with a random set of negative or positive follower traits that can't be tweaked, there are aspects that can be changed. You'll be able to change both the name, skin, and color of your chosen follower, plus some added details like facial markings.

To start, you'll only have very few skins to choose from, or you can simply keep the ones they come with. However, if you want to unlock more, there are lots of different ways. Some can be unlocked randomly found in chests or bought from stores, while other skins have very specific unlock methods. See the list below to learn more:

When crusading through each of the four areas, any chests or merchants you find that have Follower Skins will first pull from the pool of available ones for that region. They often in appear in large chests, and are also sold by the cat merchant Forneus who can appear in a room marked by a hut with a cat icon when choosing your path in a Crusade. In addition to this, you'll face four minibosses in each region on your way to the main boss of that area, and each can be converted to unlock their character skin.

However, if you don't want to leave things up to chance or grinding for random follower skin drops, you can also visit the four unlockable points of interest you'll be pointed to during your crusades in each area: Pilgrim's Passage, Spore Grotto, Smuggler's Sanctuary, and Midas's Cave. Each of these areas holds a large shrine that you can interact with to buy one of the remaining follower skins for that associated area (Darkwood, Anura, Anchordeep, and Silk Cradle). However, once you have all the ones considered to be random drops for that area, you cannot buy anymore.

Of the General Follower Skins, they can appear randomly in chests or bought from Forneus, but can't be reliably bought elsewhere. There are also a few specific Follower Skins that must be unlocked via specific means, and you can see more details for each one below:

The Rat Follower Skin is one of the earliest skins you can find and unlock. After entering Darkwood a few times at the direction of your mentor, Ratau, he will finally be satisfied you're on your way to building a decent cult, and will retire to a Lonely Shack and ask you to visit. You can teleport to it at the entrance to your cult.

The Fish Follower Skin isn't exactly in a specific location to pick up, but there is only one place to find it. After you've entered Darkwood about four times, you'll come across a fishy man who invites you to visit a place called Pilgrim's Passage. This place includes its own fishing minigame to catch food for your Followers.

Also located in Pilgrim's Passage is a Lighthouse, and a Lighthouse Keeper inside. He'll ask you to fix the Lighthouse by supplying a number of lumber, and will reward you accordingly. When you return later, he'll ask for more repairs in the form of 25 Crystal Shards, which can only be found attacking the environment in the Anchordeep region.

When crusading through the Anchordeep region, you'll run into a shifty man named Plimbo who will invite you to visit the Smuggler's Sanctuary area where he sells goods. As you explore the pier, you may spot a crab running around this area. Run after it and catch the critter along the pier to get the Crab Follower Skin.

Once you reach the final region of Silk Cradle, you may stumble into a sign post that unlocks Midas's Cave to visit outside of crusading. This final area is full of piles of gold, and gold-melted characters. If you search behind the gold piles at the back of the room where Midas' throne is, you can find the Star Follower Skin.

The Wolf Follower Skin can only be gained at the end of a long questline that can begin as soon as you reach Pilgrim's Passage. Visit the place in the dead of night, and look along the pier for a green crescent until you can interact with environment to spot a spooky wolf creature in robes. He has four quests for you in each of the areas you unlock, and can only be found at night by the crescent symbol hidden in each place. He'll ask some pretty crazy things of you, and you'll have to accept to further his questline.

Once you get to Midas's Cave and check the far back right for his last hiding spot, he'll ask for the ultimate sacrifice. Do this, and you'll get his last Holy Talisman piece, plus the Wolf Follower Skin.

This follower skin is easy to get - if you have the stomach for it. You'll need to craft a certain amount of Bowls of Poop to be awarded this prize, which means your followers are going to be in for a nasty surprise. This may mean you'll have to be sending many Followers to go rest, or have a lot of Camellia Flowers on hand to heal them in the sick bay. If you've unlocked the Crown Ability to eat food yourself, you can also subject yourself to this torment - as it doesn't matter who eats it. Once enough have been crafted, the Poop Follower Skin will automatically appear.

This tricky follower skin may be harder to get than most. At each of the five different places you can warp from the teleport pad at your cult, there's a hidden snail shrine located just off the side of the walkable space, and can easily get lost in the background art. Each of these requires a snail shell to be supplied, and you will have to get lucky as you go crusading to find snails crawling along the ground. They seem to appear very rarely, and usually only in Darkwood or Anura (though they seem to not always drop a shell even when killed).

This final follower skin is a nod to the developers - Mad Monster. This secret can only be revealed once you stumble upon the shop for the merchant called Rakshasa, who sells food alongside his giant snail lady. However, if you smack the snail, Rakshasa will start to get angry. Smack it enough times, and the merchant will fight you. He's pretty durable, but his attacks are easy enough to dodge.

Once you defeat Rakshasa, he'll admit defeat and promise to still do business if you lay off the beatings. He'll also give you the Mad Monster Statue, a unique decoration that you can only build one of.
