I replaced the heat exchanger tank and all associated hoses and boots several years ago on my 1993/4 Perkins Perama M30. My old tank's filler neck was partially broken off so my "radiator" cap barely stayed in place.  I reused my heat exchanger tube after cleaning without a problem. Obviously I had to remove my exhaust elbow as part of this process.  Although I expected a problem, I found my exhaust elbow to be completely open and seemingly fine.  I did reuse it and have not had any issue. 
I offer that there may be significant differences in this situation depending if your boat is freshwater or salt water. My boat has always been freshwater.
