Part 1 Clarification

There are bullet points that describe what to do for part 1.  Are these necessarily in order?  The last two points are:
  • Represent in padded matrix-vector form.
  • Reformulate the problem in standard form after step #2 (i.e., using slack variables).

Would be it be typical to do this last bullet point in order to do the second to last?   If not, I'm probably misunderstanding something.  thanks

Yes, that is correct.  Formulating the problem in standard form #2 normally comes before having the padded matrix.  You can arrive at the padded matrix without the full standard form (because this entails B, N, c, b, A, and v).

To re-iterate what Chris says,

Padded matrix form is a way of representing standard form step #2, so you would typically write it AFTER deriving standard form step #2. However, it is so trivial to arrive at padded matrix form by simply padding your original matrix/vector form in the right places, that the order isn't terribly important. Make sure you label your pieces clearly and unambiguously whichever order you choose.
