Hi, ladies! Please help me to write in Hungarian a letter to the school dentist that my son does not need any treatment because  we go to another dentist. In Hungarian it is called  "szülői igazolás". 
Here is in Hungarian what is needed "pénteken  fogászatra megyünk. Kérjük,akinek külön fogorvosa van,hozzon orvosi,v. szülői igazolást!"
