camp payment/medical forms- PLEASE READ

Hello Troop,
Just a reminder that payment for summer camp was due MAY 2nd. Please get that payment to me ASAP. If you are using scout account money you MUST fill out a scout check. I've attached a copy of the check to this email. I have a list of scout account balances, if needed. If you have any questions, please let me know.
Medical forms are due no later than May 26th. They will be taken to camp on June 1st and I need time to review them. Please start this process, the form has many little spots that can easily be missed. See below for details.
*Please give me two copies of all medical forms and insurance card.
*You can access an editable PDF at www.scouting.org/health-and-safety/ahmr/ I would highly recommend completing this form digitally and saving it. It makes completing each year far more easy.
*Click on the form for CAMP - it is a 4 page document (Parts A, B, B, and C) Yes, part B has 2 pages.
*You MUST enter all immunizations on the form! You can not simply attach a copy of immunizations from your physician.
*You will also need to make a copy of your insurance card - front and back
*If you have a scheduled physician visit, please let me know that date. In the meantime, give me parts A, B, B and the insurance card.
Forms can be brought to my house (46 Reddington Landing) or can be dropped off at Centre school. Feel free to send in with younger siblings, just put in an envelope with my name.

Happy Mother's Day!
We are at the point where there is no more time to wait to
pay the final camp fee. I will be going to Daniel Webster Council
on Wednesday morning to make to final payment. Payments after the
15th incur an additional $40 charge. So bringing in your money,
check, or Scout Check on Thursday will not meet the early payment
discount. You can get the payment to me, Deanne Durant, or Jen
Rodgers
For our two Scouts who joined after the initial deposit was due, I have worked it out to get you the $410 rate which is the same total as for those who paid the deposit.
