Hello Troop,

I am collecting medical forms for everyone - adults included. Please start this process now, it takes time to complete the form and get it signed by your physician. PLEASE read over the form carefully, there are quite a few areas that must be completed that can be easily missed. If you miss a section, I will highlight and get it back to you. This takes some time to complete the process, so it's a great help to get started now!

*Please make 2 copies of the completed form - one for us to keep in a binder for future outings, and one that will be sent to camp.

*You can access an editable PDF at www.scouting.org/health-and-safety/ahmr/

*Click on the form for CAMP - it is a 4 page document

*You MUSTenter all immunizations on the form! You can not attach a copy of immunizations from your physician.

*I would highly recommend completing this form digitally and saving it. It makes completing each year far more easy.

*You will also need to make 2 copies of your insurance card - front and back
