Welcome, everyone to Mayo Clinic Q&A. I'm Dr. Halena Gazelka. Soft tissue sarcomas are rare forms of cancer that have been typically treated with what has been called limb salvage surgery combined with radiation therapy. While limb salvage surgery helps patients avoid amputations, patients are often left with substantial functional limitations. Now advancements in microsurgery are making it possible to harness the body's own ability to regenerate muscle strength after surgery that is performed to remove soft tissue sarcomas. This is a process called oncoregeneration, and it's our topic for today. Joining us to discuss our Mayo Clinic orthopedic surgeon, Dr. Matthew Houdek, and plastic surgeon Dr. Steven Moran. Thank you for being here today gentlemen.

Our thanks to Mayo Clinic orthopedic surgeon, Dr. Matthew Houdek, and plastic surgeon, Dr. Steven Moran, for being here today to talk to us about sarcomas and the use of oncoregenerative surgery to manage them. I hope that you learned something, I know that I did. We wish each of you a very wonderful day.

Dr. Adam Brown: Usually, the patient will tell you. They'll say, "Hey, look, I tried the injection, I tried the pills, this is driving me nuts." Then, we work with my orthopedic surgery colleagues and they help us decide that.

This is the most common surgery performed for Dog ACL surgery. Actually, there are many different names for this procedure, but these are just a few of the most common. Essentially the concept for the surgery is very simple, to stabilize the knee on the outside of the joint by using a single fiber plastic line called a mono-filament. This very strong suture or the line outside of the joint re-establishes the stability of the joint needs when the ACL is torn. This suture is essentially placed in the same orientation or plane that the original ACL ligament was, except it is placed on the outside of the joint. During the surgery, a hole is first drilled through the front part of the tibia (shown above). Following this, the suture is looped around the small bone called the fabellar bone (also shown above) on the backside of the femur and continued alongside the knee, through the drilled hole in the tibia, and then it is clamped together with a stainless steel clip.

After rotator cuff repair the patient needs to plan on being less functional than usual for twelve or more weeks after the procedure. The shoulder should not be used with the elbow away from the side for 3 months after a rotator cuff repair. Lifting pushing pulling and many activities of daily living place stresses on the rotator cuff and can place excessive tension on the cuff repair risking failure of the repair. Driving shopping and performing usual work or chores may be difficult during this time. Plans for necessary assistance need to be made before surgery. For people who live alone or those without readily available help arrangements for home help should be made well in advance.

When not diagnosed early and managed appropriately, common musculoskeletal injuries may result in long-term disabling conditions. Anterior cruciate ligament tears are some of the most common knee ligament injuries. Slipped capital femoral epiphysis may present with little or no hip pain, and subtle or absent physical and radiographic findings. Femoral neck stress fractures, if left untreated, may result in avascular necrosis, refractures and pseudoarthrosis. A delay in diagnosis of scaphoid fractures may cause early wrist arthrosis if nonunion results. Ulnar collateral ligament tears are a frequently overlooked injury in skiers. The diagnosis of Achilles tendon rupture is missed as often as 25 percent of the time. Posterior tibial tendon tears may result in fixed bony planus if diagnosis is delayed, necessitating hindfoot fusion rather than simple soft tissue repair. Family physicians should be familiar with the initial assessment of these conditions and, when appropriate, refer patients promptly to an orthopedic surgeon.
