Automatic curly quotes in Bruce

I added smartypants integration to bruce, to automatically curl quotes.
It gives a more polished look to the presentation. Attached is a diff.
I don't know if this is the kind of thing that needs to be an option,
Richard can decide that (if he chooses to include it in bruce at all!).
It curls quotes in titles and non-literal text, and requires smartypants
to be installed (http://web.chad.org/projects/smartypants.py/).

Diff applied with a few changes:

- added a command-line option to allow fine control and turn it off
(it's reasonable that there could be a stylesheet control)
- made qbD the default because of personal preference ;)
- some other stuff
