WIC is now accepting nominations for WIC Everyday Heroes. The MN WIC Advisory Group established the WIC Everyday Hero award to recognize the everyday heroes working in WIC at the local level. Nominations are being accepted through Friday, June 12. Each WIC local agency may submit up to two nominations. Each of the eight WIC regions will select one Everyday Hero from their region. The Center for Health Promotion at the Minnesota Department of Health has released a grant Request for Proposals (RFP) to support community-based solutions for healthy food access and diabetes prevention and management. Proposed projects should address food access at a systems or population level versus at an individual level. This RFP seeks project proposals to address barriers to accessing healthy foods or understanding and accessing nutrition information, education, or support services. These grants will run for about two years with a range of $25,000 - $37,500 annually. Applications are due July 1, 2026. Selected organizations must primarily and intentionally serve people who experience food and nutrition insecurity, with an emphasis on reaching populations at higher risk of prediabetes or diabetes and hospitalization/morbidity risk, including at least one of the following populations: • U.S. born and other African American/Black communities • American Indian communities • Asian/Pacific Islander communities • Hispanic/Latino/Latina/Latine communities • People living with disabilities • Greater Minnesota Please share with your networks!
The Request for Proposals (RFP) for 2026 is now open through Friday July 19th at 4:30 p.m. for Private Well Protection Grants. The Minnesota Department of Health (MDH) received funding from the Clean Water Fund to protect private well users. A portion of this funding is being offered as grants to promote private well testing among private well users. Each entity will have the opportunity to apply for a set amount of funding to provide and promote well testing for households that rely on private well water for drinking water. Eligible applicants include Community Health Boards/Local Public Health, County Units of Government, and Tribal Governments.

The Minnesota Community Health Worker Alliance invites proposals for the 2026 Rural Community Health Worker Conference to be held October 8th and 9th in Worthington, Minnesota. The theme for this year’s conference is "Hands On, Hearts In: Practicing the Skills That Power Rural CHW Work". This fully in-person conference will have approximately three Plenaries (1 hour), twelve breakout sessions (approximately 45 minutes) and up to 20 poster presentations. Apply now through June 30th to present at this conference that will bring together CHWs, CHW Certificate students, CHRs, Promatoras, CHW employers, public health professionals, heath providers, policy makers, payers, academic and training partners. Organizations in Minnesota are invited to apply for a $25,000 opportunity to strengthen community and clinical linkages (CCLs) for type 2 diabetes prevention and management. This year long project, led by Mathematica in partnership with the Minnesota Department of Health, will support programs serving African American, American Indian, and rural communities. Otter Tail County Public Health provides population-based public health nursing services that promote, protect, and improve the health of individuals, families, and communities. This position assesses community health needs, emphasizes prevention and social determinants of health, collaborates with community partners, and supports public health initiatives. This includes advocacy, policy development, and health planning activities in alignment with public health standards and community health priorities. This position is part of the Long-Term Care Unit and is assigned to the Interdisciplinary Public Health/Human Services Unit and includes, but is not limited to, providing care coordination to persons who are enrolled in managed care health plans. Care coordination includes assessment, care plan development, arranging and/or coordinating supports and services which may include preventative, routine, and/or long-term care health and service needs.

Public Health Nurses focus on promoting healthy families and communities; supporting disabled and senior persons to live independently; preventing and responding to diseases and health threats; and identifying and preventing environmental health hazards.

Des Moines Valley Health and Human Services (DVHHS) currently has a full time opening for a Public Health Nurse or Registered Nurse. The Public Health Nurse or Registered Nurse provides skilled nursing care to assess physical, emotional and medical needs of families, carries out established plans of care, updates records, completes documentation of care, assists families in achieving goals, coordinates with school personnel and refers family health clients to community resources. Duties also include providing client/family assessments, teaching and coordination, screening, monitoring for complications and assisting families/clients to recognize early signs of disease and deviations from normal health. The Public Health Nurse/Registered Nurse position is 40 hours per week with a full benefit package which includes Public Employee Retirement pension and is classified with a full pay range of $29.71 - $41.12 per hour.

Provides servicesin a variety of settings that include home visits, office, and facilities. Supports improved health outcomes for individuals, families, communities and populations. Required Special Qualifications: Licensure as a Registered Nurse in the State of Minnesota is required upon employment. Certification as a Public Health Nurse in the State of Minnesota is required within six (6) months of employment.
The Health Educator identifies health education needs of the population of Rice County, and implements effective community-based health promotion strategies to address identified needs with a focus on policy, systems, and environmental change. Health promotion strategies may address one or more of the following areas: nutrition, physical activity, tobacco prevention, access to health care, mental health and wellness. The Health Educator engages in activities such as community health assessment, strategic planning, policy/ordinance development, community health education both in-person and written, health related social media postings, and grant implementation and evaluation. The Health Educator in this position will serve as the Rice County Public Health Statewide Health Improvement Partnership (SHIP) Coordinator.
Required qualifications: Bachelor’s Degree in Nursing, Community Health, Health Education or a health-related field and one to three years of public health education, or equivalent. Valid driver’s license. Preferred qualifications: Previous work experience in public health setting. Bilingual skills of English/Spanish or English/Somali.

The purpose of this grant funded position is to provide leadership for the supervision and coordination of activities related to public health emergency preparedness. Success in the position will ensure development, coordination, and delivery of planning and response measures and continuity of operations within the City and the Health Department.

Minimum experience: A minimum of three years’ experience in the areas of public health, public safety, and project required.
