Reminder: Annual Immunization Status Report Due Dec. 1 October 22, 2025  Annual Immunization Status Report due Dec. 1 Please make sure the school nurse for your institution receives the message below.  This is a reminder that all K-12 schools need to complete the Annual Immunization Status Report (AISR) by Dec. 1. The School Health Personnel Immunization and Disease Reporting website is organized to provide you with the information you need to implement the Minnesota school immunization law, report diseases, and provide information to students and families. For instructions on completing the AISR refer to the user guide found under the Complete the Annual Immunization Status Report (AISR) tab.  We need you to:  Complete the AISR by Dec. 1 by visiting Welcome - AISR (state.mn.us).  If you are not able to meet the deadline you can contact us at healt...@state.mn.us, and we will work with you directly to provide an extension.   Questions?  Contact the Immunization Program at health.aisr@state.mn.us or 651-657-5503 or 1-800-657-3970.  Thank you for collaborating with MDH to ensure compliance with our school immunization law.
