Hello,
 
I hope this email finds you well; my name is Melody Schaeffer and at the Child Health Advocacy and Outreach Department of St. Louis Children’s Hospital. The Hospital is currently conducting a Community Health Needs Assessment, as required by the IRS. To complete this assessment, we are collecting data from parents living in the St. Louis Metro region through an electronic survey. I have included a brief overview of the initiative below.
 
We would thoroughly appreciate it if there was a way you could distribute this survey along to your contacts. Again, this survey will allow parents to rate their health concerns (from social determinants of health to clinical diagnosis) for their community. There is no need to be a patient of the Hospital to take the survey, we are more concerned with getting a representative sample of individuals and their specific health needs
 
If you would like, I can send along a PDF version of the survey, however, we prefer that it be distributed electronically.
 
When it comes to the children of this community, few of us could argue that they are our most precious resource.  St. Louis Children’s Hospital has a deep commitment to improving the lives of our youngest citizens. 
St. Louis Children’s Hospital is conducting a Community Health Needs Assessment to identity the most important pediatric health issues in the St. Louis region. This survey will assess parents’ health concerns for their children and for children in the community. The survey will ask the respondent to rank different items on a four-point scale of how much of a problem they are in the community (Big problem, Medium problem, Small problem, Not a problem). This survey will only take 5 minutes to complete. Additionally, the survey is completely confidential and we will have no idea (other than ZIP) where someone lives, or who they are.
 
Our goal is to get 1,000 responses.
